CARE MANAGEMENT: The Area II Agency on Aging Care Management Program is designed to support independent living in the community. Services are individually prescribed and arranged according to the medical and psychosocial needs of the recipient. An individual plan of care is developed by the Care Management Team in conjunction with the recipient and the attending physician.


INFORMATION AND ASSISTANCE: Provide information to older persons, disabled and their families about services available in the community. Make referrals to agencies which can either answer their questions or provide the services they need.


CERTIFIED LOCAL OMBUDSMAN PROGRAM (CLO): Advocate for residents in long term care facilities and personal care homes. Receive and investigate complaints made by or on behalf of residents and work for a resolution.


STATE HEALTH INSURANCE COUNSELING ASSISTANCE PROGRAM (SHIP): Provide information to Medicare beneficiaries about their health insurance options and rights. Assistance with Medicaid, SSI, QMB/SLMB and long term care insurance.


CONTRACT with senior citizen centers and other service providers in an eleven county area to provide: Congregate Meals - Home Delivered Meals - Homemaker - Health Screening - Transportation - Senior Center Activities - Home Health Aides - Personal Care Attendants - Skilled Nursing Service - Respite Care - Outreach - Legal Services. These services are available to those aged 60 or over and their spouse without regard to race, color, national origin, handicap or age.
